Analysis of influencing factors and impact path of compassion fatigue in nurses from maternity hospitals in Jiangsu

Objective To investigate the influencing factors of compassion fatigue(CF) among nurses in maternity hospitals,and then to further explore the impact path of factors. Methods 715 nurses in maternity hospitals from Jiangsu were investigated by the cross-sectional survey. A self-developed questionnaire,the Five-Factor Inventory,the Social Support Rate Scale,the Psychological Capital Questionnaire-Revision,and the Compassion Fatigue Scale were adopted to collect data. Results The number of children,personality of neuroticism and agreeableness,social support,and psychological capital are the influencing factors of CF(P<0.05),while professional titles,length of services,labor relations,personality of extraversion and openness,social support and psychological capital influence compassion satisfaction(CS)(P<0.05). In structural equation model,the indirect effects of neuroticism on CF or CS existed(ES=0.062,-0.420). Social support and psychological capital play a partially mediating role in neuroticism and CF(ES=0.017),accounting for 27.42% of the total indirect effects. They also play a completely mediating role in neuroticism and CS(ES=-0.112),accounting for 26.67%. Positive personality had a significant indirect effect on CF or CS(ES=-0.158,0.623),and social support and psychological capital serve as a completely mediating role between them (ES=-0.028,0.102),accounting for 17.72% or 16.37% of the total indirect effects. Conclusion Nursing managers should take effective measures tailored to personality traits to help nurses meet high levels of social support and psychological capital,so as to promote CS and reduce CF.

Evidence summary of non-pharmacological interventions for maternal postpartum depression

Objective To retrieve,evaluate and summarize the best evidence of non-pharmacological interventions for maternal postpartum depression and to provide references for clinical practice. Methods According to the top-down principle of the evidence-based resource 6S model,a systematic search of domestic and foreign databases and websites was conducted on the intervention of postpartum depression,including relevant clinical decision reports,best practices,guidelines,evidence summaries,systematic reviews,and expert consensuses. The search time limit was from the establishment of the databases to March 2022. A supplementary search was performed for randomized controlled trials published between September 2019 and March 2022. There were 2 researchers who evaluated the quality of the literature,extracted and summarized the evidence according to the subject. Results Totally 15 articles were involved,including 1 best practice,3 guidelines,2 expert consensuses and 9 systematic reviews. Finally,28 pieces of best evidence about iatrophysics,psychological interventions,exercise interventions,other interventions,and intervention implementation and monitoring were summarized. Conclusion Non-pharmacological interventions for maternal postpartum depression have various forms and rich contents. Appropriate intervention plans should be made according to the actual situation and the willingness of parturients,and the process of interventions should be well monitored.

The real experience of hypertensive disorders of pregnancy in pregnant women:a qualitative meta-synthesis

Objective To systematically evaluate the disease experience and psychological feeling of pregnant women with hypertensive disorders of pregnancy,and to provide the evidence for disease management measures for pregnant women with hypertensive disorders of pregnancy. Methods Chinese and English databases,including SinoMed,CNKI,Wanfang,VIP,PubMed,Cochrane Library,Web of Science,Embase,PsycINFO,Proquest,CINAHL,were searched for qualitative studies on real experience of hypertensive disorders of pregnancy in pregnant women from the inception of databases to November 10th 2021. The quality of included studies was evaluated according to JBI Critical Appraisal Tool for qualitative studies in Australia. The results were integrated by integrating methods. Results 16 studies were included in the final review. Totally 60 findings were extracted;12 new categories were generated;4 integrated results were synthesized as following,complex and lasting emotional response,breaking the pregnancy norm,seeking understanding and support,finding the benefits from experience. Conclusion Nurses should strengthen the dynamic assessment of the mental health of pregnant women with hypertensive disorders of pregnancy,promote the establishment of a multi-coping system of medical care-family-self through coordinated and continuous maternal health care services,enhance the sense of pregnancy control of such pregnant women,reduce the adverse effects of disease factors on pregnancy outcomes and mental health,and ensure the safety of mothers and infants.

Disease communication experience between couples in patients with colorectal cancer stoma:a qualitative study

Objective To explore the problems and causes of disease communication between colorectal cancer patients and their spouses,and to provide a basis for further interventions.Methods Purposive sampling method was used to select patients with colorectal cancer from September to December 2019 in a tertiary hospital in Zhengzhou for semi-structured interviews. Data was analyzed by Colaizzi method.Results 4 themes were extracted. Theme 1 is negative communication problems during the treatment(including heavy communication burden of male patients,the lack of emotional support in communication content,couples avoidance of cancer-related topics). Theme 2 is positive communication experience in the rehabilitation period(including positive communication about the new life after illness,the increasing of the sense of tacit understanding). Theme 3 is the influence of negative communication on patients(including the increased negative emotions,the increased inner loneliness of patients,the decline of self-confidence,the conflicts caused by over protection of spouses). Theme 4 is patients’ expectation of effective communication between the husband and wife(including the obtaining of professional knowledge guidance to improve communication efficiency,the hope to get training in communication skills).Conclusion There are differences in disease communication of couples in different stages of ostomy patients with colorectal cancer. Negative communication will have a negative effect on the patient’s psychological status. Therefore,nurses should pay attention to such patients and give effective interventions.

Effects of anxiety about pregnant women in the first and third trimesters on the delivery modes

Objective To explore the effects of anxiety on pregnant women in the first and third trimesters on delivery modes,in order to improve the labor outcomes. Methods Maternal data from November 2017 to June 2019 in the Obstetrics and Gynecology Hospital of Fudan University information system were extracted after psychological assessment of pregnant women in the first or third trimester. An anxiety group and a non-anxiety group were divided according to the Generalized Anxiety Disorder Scale(GAD-7scale),and the delivery modes of 2 groups with propensity score matching were compared. Results After propensity score matching,447 pairs of anxiety groups and non-anxiety groups in early trimesters and 53 pairs in late trimesters were successfully matched;there was no significant difference in delivery modes between anxiety groups and non-anxiety groups in early trimesters( χ 2=1.620,P=0.203);there were significant differences in delivery modes between anxiety groups and non-anxiety groups in late trimesters( χ 2=4.330,P=0.037). Pregnant women in the anxiety group tended to choose cesarean section. Conclusion Results showed that anxiety in the first trimester had no effect on the choice of delivery modes,but anxiety in the third trimester might increase the incidence of cesarean section. It is suggested that cesarean section rates can be reduced by alleviating anxiety in the third trimester,so as to improve the labor outcomes.

Construction of a nursing management model in a makeshift(Fang Cang)hospital under the epidemic of coronavirus disease 2019

Objective To construct a nursing management model of a makeshift(Fang Cang) hospital under the epidemic of coronavirus disease 2019(COVID-19),so as to provide the experience and references for the establishment and the nursing management of the makeshift(Fang Cang)hospital. Method With the reference to the Jennings Disaster Nursing Management Model,we combined with the specific situation,formulated corresponding measures,connected all links,and finally constructed the nursing management model of the makeshift(Fang Cang) hospital under the epidemic of COVID-19 based on the in-depth analysis of the characteristics of nursing tasks in the makeshift hospital. This nursing management model includes 4 periods,namely the initial stage,the normal operation period,the closed period and the evaluation period. Results Under this model,the Jianghan makeshift(Fang Cang)hospital has been in effective operation for 34 days with a total of 1 564 beds. A total of 1848 patients were admitted and treated,with 1 327(71.81%) being cured and discharged and 521(28.19%) being transferred,and with no death. 675 nurses participated in the rescue aid with no infection. A total of 5,020 nucleic acid tests and 2,409 CT examinations have been completed,which made Jianghan makeshift(Fang Cang) hospital the largest hospital with open beds,cumulative admissions and discharge,nucleic acid monitoring and CT examination. Conclusion The establishment of the makeshift(Fang Cang)hospital under the epidemic of COVID-19 can effectively deal with public health emergencies. A highly effective nursing management model can help with the smooth operation of the makeshift(Fang Cang)hospital,which plays an important role in the combat against the epidemic.
